On a voluntary basis, you may elect to take an additional Voluntary Term Life Insurance policy. If you enroll within 31 days of benefit eligibility and meet the active work requirement, you will automatically qualify for up to a set amount of insurance coverage called the "guarantee issue amount."

Voluntary Life Booklet (with enrollment form) 

You may pay for Vountary Short-Term Disability (VSTD).  The STD plan will replace up to 60% of your weekly income to a maximum of $1,000.  This coverage lasts up to 90 days, or when the LTD begins to pay, whichever is earlier.

Voluntary Short-Term Disability Booklet (with enrollment form) 

You may pay for additional AD&D Coverage. Coverage is completely voluntary, and you can enroll at any time.
