Community Health Improvement Plan Workshop Flyer

The Community Health Improvement Plan (CHIP) is a community-driven plan that turns the priorities identified in our Community Health Assessment (CHA) into action. TCPHSS is the facilitator and supporter of the CHIP, but a successful health improvement process requires relationship building across sectors, broad community engagement, collaborative action, and comprehensive assessment of our needs and strengths.
